Navicore Solutions through the NJ Housing Supports Program offers personalized assistance to eligible individuals and families to help them find, secure, and maintain safe and affordable housing. Services include housing search support, landlord coordination, budgeting and financial education, and ongoing case management.

Frequently Asked Questions

You need to meet three basic criteria:

  1. Be enrolled in Medicaid with a Managed Care Organization (MCO).
  2. Have one clinical risk factor, like: chronic health condition, mental health condition, substance misuse, pregnancy, complex medical health condition caused by an intellectual or developmental disability, repeat use of emergency room/multiple hospital admissions.
  3. Have one social risk factor, like: currently homeless, at risk of being homeless, recently released from a correctional facility, transferring from a long-term mental health or substance use treatment center.

Housing Supports Services will be covered by Medicaid Starting July 1, 2025. You can sign up by clicking on the button above and scheduling an intake appointment.

Pre-Tenancy services cover three primary areas.

  1. Develop an Individualized Housing Plan to help you establish both short and long term goals to secure housing.
  2. Assist with navigating the housing application process to include searching for housing, talking with possible landlords, help with rental applications and the tenant screening process.
  3. Identifying, coordinating, and securing resources to assist with housing costs and other expenses.

Tenancy Sustaining Services helps with things like:

  • Updating/revising your Individualized Housing Plan once you are in your new home.
  • The housing recertification process including lease renewals and housing subsidy renewals.
  • Understanding the rights and responsibilities of the tenant and landlord

 

Move-In Supports covers one-time expenses, like:

  • Purchase of household furnishings including furniture, food preparation items, pantry stocking, or bed/bath linens.
  • Costs for filing applications and payment of application fees
  • Move-in costs including movers.
  • Payment of security deposits.
  • Payment of set-up fees or deposits for utility or service access, including telephone, electricity, heating and water.

 

Monthly rent and the provision of, or payment for, housing vouchers is not covered.

There are a few limitations for each type of service offered.  They are:

  • Pre-Tenancy and Tenancy Sustaining Services have no cap on how long you can receive these services, but they need to be reauthorized every six months. You cannot have more than one provider of these services or receive Community Service Supports (CSS) at the same time.
  • Move-In Supports
    • Lifetime Cap of $10,000.
    • One-time benefit – you are limited to one “moving experience” per lifetime.
    • Does not allow: transitions into institutions (i.e. nursing facilities), residential or vehicle modifications, recreational items, pre-owned items that may be physically unsafe, recurring expenses (i.e. food, utilities).
    • If you are receiving MLTSS Residential Modifications and Remediation Services, you may not combine or add spending caps across the two programs.
